• Initial Comprehensive Assessment:
  • In-depth initial assessment, including a detailed health history review, baseline biometric measurements, and an initial consultation with a healthcare professional.
  • The care team will determine if the clinic will meet the needs of the patient and discuss the patient’s success plan and recommend a membership level.
• Follow-up Visit $200
• Membership Required after 2nd visit, it also can start at the 2nd visit.

Basic Metabolic Wellness Membership: $149/month

• Access to educational webinars on functional medicine and metabolic health.
• Exclusive resources, articles, and recipes for members.
• Priority scheduling for routine check-ups and consultations.
• Annual Executive Physical including a wellness and preventive plan.
• Unlimited Visits with medical care team (NP, PA, Pharmacist), most patients will use 6-8 visits a year.

Premium Integrative Care Membership: $229/month

• Includes all Basic Wellness benefits.
• Quarterly personalized metabolic health assessments.
• Direct access to a dedicated health coach for ongoing support. Health coaching visits may be virtual.
  • Includes one 1:1 coaching or nutrition visit per month
  • Pricing varies by location.
• Unlimited visits with medical care team (NP, PA, Pharmacist) (most patients will use 6-8 visits a year).

VIP Total Transformation Membership: $349/month (Only available in Centerville, Ohio)

• All benefits from Basic and Premium memberships
• Plus 2 Fitness classes per week at Kelli’s Body Shop to help your transformation! Classes for all types are available, including classes for 55+.

We do strongly encourage that each patient have a separate primary care provider for acute care needs that may arise. The primary focus of our office is metabolic wellness and chronic care management. We cannot guarantee availability for acute visits.
